This patch updates the unit test for get_lostreturn_policy to reflect
the rule_name and return code changes.
Test plan:
1/ Read the code changes in t/db_dependant/Koha/CirculationRules and
t/db_dependant/Koha/IssuingRules and confirm they make sense.
2/ Run the updated tests and ensure they pass.

Test plan:
Apply only the first patch, run the tests and confirm that the failures
make sense
Apply this patch and confirm that the test now returns green

We used to test rule scopes by explicitly defining each combination.
When adding new scopes, it is much easier if these tests are auto-
generated for you so that you don't have to repeat similar code.
This patch removes those "duplicates" and adds a method that returns
test cases for each scope as follows:
branchcode categorycode itemtype
__________ ____________ ________
branchcode categorycode itemtype
branchcode categorycode *
branchcode * itemtype
branchcode * *
* categorycode itemtype
* categorycode *
* * itemtype
* * *
And automatically extends the test when new scopes are added.
This also obsoletes the "Get effective issuing rule in correct order"
test in t/db_dependent/Koha/IssuingRules.t
To test:
1. prove t/db_dependent/Koha/CirculationRules.t

Unfortunately, the tables here can't use a foreign key as one table uses null where the other uses
'*' This patchset alters the delete method so delete related rules. It is somewhat of a workaround until
all the columns in issuingrules are moved to circulation_rules
To test:
1 - Add some issuing rules in koha, making sure to set maxissueqty
2 - Check the DB: SELECT * FROM circulation_rules;
3 - note some circulation rules were created
4 - Delete your rules via the staff interface
5 - Check the DB, the circulation rules remain
6 - Apply patch
7 - Repeat
8 - Huzzah! The rules delete!
9 - Prove the tests, read the code

sub in IssuingRules.t too.

This patchset move The OPACItemHoldsAllowed logic
(issuingrules.opacitemholds) to a new class method of
Koha::IssuingRules: get_opacitemholds_policy
On the way, this patch will certainly fix the same problem as bug
19298 with onshelfholds.
Test plan:
Make sure the opacitemholds policy is correct when placing a hold at the
OPAC or the staff interface.

git grep '^1;$' t/**/*.t
should not return any results

When calling the proposed version of get_effective_issuing_rule with undefined
parameter values, a following crash occurs:
SQL::Abstract::puke(): [SQL::Abstract::__ANON__] Fatal: SQL::Abstract before v1.75
used to generate incorrect SQL when the -IN operator was given an undef-containing
list: !!!AUDIT YOUR CODE AND DATA!!! (the upcoming Data::Query-based version of
SQL::Abstract will emit the logically correct SQL instead of raising this
exception) at /home/ubuntu/kohaclone/Koha/Objects.pm line 182

This test prints the amount of issuing rule matches per second for
1. worst case, when non-existent branchcode, categorycode and itemtype is
being searched (currently 8 queries)
2. mid case (rule found on 4th query)
3. 2nd best case (rule found on 2nd query)
4. best case, when an issuing rule is defined for exactly those branchcode,
categorycode and itemtype (currently 1 query)
To test:
1. Run t/db_dependent/Koha/IssuingRules.t
2. Write down the per-second amount to compare with next patch
